1. Environment

Obesity tends to run in families. However, children with no prior family history of obesity are increasingly becoming overweight and/or obese. Some socioeconomic factors that are associated with obesity and overweight are lower socioeconomic means (low food quality), unemployment, and ethnicity (Akil & Ahmad, 2011).
3. Genetics
Obesity-related genes do not mean that one is destined to be obese, but there is a tendency for higher risk. However, there are rare genetic disorders that do directly cause obesity, such as both Bardet Biedl and Prader-Willi syndromes (Research Chair in Obesity, n.d.).
4. Metabolism
Mechanisms that regulate satiety and hunger are being studied further to determine a consensus about the actual effects on weight. While there is such a thing as slow or low metabolism, it is rare, and usually not what causes one to be overweight or obese (Hensrud, Mayo Clinic, 2015).
5. Entrenched behaviors or habits
Snacking at certain times, eating to relieve stress, eating when depressed, eating in a hurry, and overeating all typically contribute to the consumption of empty, added calories.
The Centers for Disease Control and Prevention (2018b) suggest using the following 5-Step guide when assisting those who want to start losing weight. This guide aligns perfectly with the use of motivational interviewing within NSHC's context of health coaching.

Assessment :
A client's measurements and co-morbidities serve as the baseline from which to measure weight loss and weight maintenance goal achievement. There are four primary elements of the initial weight assessment. These are:
• Readiness to lose
• Height and weight
• Body Mass Index
• Waist circumference
• Co-morbidity risk
1. Readiness to lose/Height and weight
We have Readiness for Change Tool(s) as one way to assess one's readiness to lose. Client's "confidence level” to lose and the “importance" that he or she places on weight loss are critical factors that will help to determine this. The initial weight assessment should also include:
• Reasons and motivation for weight loss
• Previous weight loss attempts
• Family and social support, particular challenges faced
• Attitudes about adjusting food intake and physical activity.
2. Height and Weight Measurements
If a client has scales that are accurate, have the client weigh first thing in the morning, after urinating, before eating, and only in very light clothing/undergarments with no shoes. Scales should be calibrated for accuracy and should be placed on a level non-carpeted surface.
3. Body Mass Index (BMI)
BMI is a formula used to indicate an optimal weight. It's a simple index of relative weight to height that is commonly used to classify overweight and obesity in adults, regardless of gender. It is defined as weight in kilograms divided by the square of height (WHO, 2018b).
WHO defines overweight as having a BMI ≥ 25 and obese when the BMI ≥ 30.

Because BMI is more often used as a measure of obesity rather than undesirable weight, it can be misleading. BMI doesn't take into account distribution of body fat. For example, two men weigh 180 lbs. One is 5'3" tall and the other is 6' tall. One has a "pot belly" with thin arms and legs, while the taller one has an acceptable waist measurement. Let's look at the role of waist circumference.
4. Waist Circumference
There is a direct correlation with the amount of visceral fat in relation to the development of chronic conditions. Those with increased waist circumference (men >102cm or 40 inches and women with >88cm or 35 inches) have an increased risk of developing a chronic condition, such as coronary heart disease, type 2 diabetes, hypertension, and/or increased cholesterol. Waist circumference is considered a more accurate measure of undesirable health risk.
The simplest way to measure the waistline is to take a tape measure and put it around the waistline at its smallest part, usually just above navel.
High Risk: Women: >35 inches; Men: >40 inches
5. Co-Morbidity Risk
Assess present conditions and history, as well as family history. A 5% - 10% weight loss can reduce the risk of heart disease and diabetes and should be encouraged for all overweight/obese clients (Wing, Espeland, Clark, Hazuda, Knowler, Pownall, Unick, Wadden and Wagenknecht, 2016).
